This opportunity is located at our Stop 11, Family Medicine Practice, at 5908 E. Stop 11 Road Indianapolis, IN 46237.

Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Stop 11, Family Care Medicine practice.

• Family Medicine Practice inclusive of all age ranges

• Single, robust electronic medical record system (EPIC)

Experience

• Family Medicine experience as a Nurse Practitioner is preferred

• Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.

• Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
