The Advanced Practitioner will provide Primary and Post Acute Care including: Medical management for Primary Care Office and acute/episodic, rehabilitation patients, as well as long term chronically ill patients. Some weekend call availability is required. The practitioner will provide ongoing medical management for primary care clinic located on campus as well as those in the sub- acute rehabilitation facility, long term care and memory care areas.
