In this role, you will conduct annual risk assessments and chronic condition reviews, primarily performing assessment tasks without the need for prescribing or treatment. The position emphasizes telehealth (anticipated 70%) and home health visits (30%), involving scheduling within a 30-mile radius of your assigned address. The role ensures you’ll see both pediatric and adult patients, with a manageable schedule of about 10 visits daily if performing a mix of telehealth and home visits.

This position offers a full-time schedule, Monday through Friday, 8 am to 5 pm, with a 1-hour lunch break. Equipment necessary for the role will be mailed to you, and scheduling is released 2-3 weeks in advance to allow time for familiarization with patients. Paid orientation of 1-3 hours is included, with ongoing education completed during downtime within working hours. The role requires you to be enrolled and credentialed in Medicard in advance.

Key Skills & Qualifications:

• Valid Michigan Medical License (Active)
• Current Nurse Practitioner Certification
• Experience or comfort with pediatric and adult patient populations
• Excellent communication and interpersonal skills
• Ability to work independently and manage a flexible schedule
• Proficient with telehealth platforms and electronic health records (EHR) systems

Nice to Have:

• Prior experience in telehealth or home health settings
• Familiarity with risk assessment and chronic disease management
• Bilingual abilities or additional language skills

Preferred Education & Experience:

• Master’s Degree in Nursing (MSN or Certified Nurse Practitioner)
• Minimum of 1-2 years of clinical experience in primary care or related fields

Other Requirements:

• Enrollment and credentialing in Medicard (please confirm during application)
• Comfort with seeing both pediatric and adult patients
• Willingness to work within the Milwaukee area for home visits
• Ability to handle both telehealth and in-home visits effectively
