Required Skills:
  • Valid Oregon Medical License and provider credentials
  • Proven experience in family medicine, pediatrics, or primary care settings
  • Proficiency in telehealth platforms and electronic health records
  • Excellent communication skills and clinical judgment
  • Ability to perform risk assessments and chronic condition reviews
  • Strong organizational skills and attention to detail
  • Ability to work independently and as part of a collaborative team

Nice to Have Skills:
  • Experience with home health care and in-home visits
  • Bilingual abilities, especially Spanish or other prevalent languages in the community
  • Familiarity with healthcare reimbursement and billing processes
  • Additional certifications in advanced practice nursing or chronic disease management

Preferred Education and Experience:
  • Master's or Doctoral Degree in Nursing (MSN or DNP)
  • Prior experience in a family practice or community health setting
  • At least 2 years of relevant clinical practice preferred

Other Requirements:
  • Availability for full-time (40 hours/week) or part-time (20 hours/week) schedules
  • Availability to work Monday through Friday, 8 am to 5 pm, with a 1-hour lunch
  • Adaptability to a schedule primarily consisting of telehealth (70%) and in-home visits (30%)
  • Willingness to travel within the designated counties; mileage reimbursed at IRS standard rates
  • Ability to complete ongoing education modules during scheduled downtime
  • Must be enrolled and credentialed in Medicard (confirmation required upon application)

Position Highlights:
  • Hourly Pay Rate: $95.48
  • Contract Duration: Starting April 13, 2026, for three months with potential extension
  • Schedule: Flexible full-time or part-time, with schedules released 2-3 weeks in advance
  • Work Environment: 12-14 telehealth appointments per day or 6-7 in-home visits; mix of both typically around 10 visits daily
  • Role Focus: Conduct risk assessments, review chronic conditions, escalate urgent findings, and support preventative health efforts
