Position Overview

We are seeking an Inpatient Nephrology Advanced Practice Provider (NP or PA) to provide consultative care for hospitalized patients with acute and chronic kidney conditions. This role focuses on delivering evidence-based nephrology care in a collaborative, fast-paced hospital environment.


Key Responsibilities

Clinical Care

  • Perform inpatient nephrology consultations and follow-up evaluations
  • Manage:
    • Acute Kidney Injury (AKI)
    • Chronic Kidney Disease (CKD) exacerbations
    • Electrolyte and acid-base disorders
    • Hypertension
  • Oversee inpatient dialysis modalities:
    • Hemodialysis (HD)
    • Continuous Renal Replacement Therapy (CRRT)
    • Peritoneal Dialysis (PD)
  • Document consults, progress notes, and treatment plans in the EHR

Care Coordination & Communication

  • Respond to consults from hospitalists, ICU, ED, and surgical teams
  • Collaborate daily with multidisciplinary teams
  • Ensure smooth transitions of care with outpatient nephrology providers
  • Partner with case management on discharge planning and dialysis placement

Quality & Operations

  • Maintain accurate, timely documentation
  • Ensure compliance with CMS, hospital policies, and dialysis regulations
  • Participate in quality initiatives (AKI management, dialysis safety, readmission reduction)
  • Support accurate coding and billing practices

Team & Education

  • Participate in inpatient service coverage
  • Provide education to residents, fellows, and nursing staff
  • Foster a collaborative, patient-centered care environment

Qualifications

Required

  • Licensed Acute Care Nurse Practitioner (ACNP) or Physician Assistant (PA-C)
  • Experience in acute care, hospital medicine, or inpatient settings
  • Strong clinical, communication, and consultative skills
  • Ability to thrive in high-acuity, fast-paced environments

Preferred

  • Experience with HD, CRRT, and PD
  • Prior nephrology or ICU experience
  • Familiarity with EHR systems and dialysis workflows
