We are seeking a compassionate, adaptable, and motivated provider who is looking to grow within a fast-paced, evolving environment.

This provider will support facilities in the Beachwood, Mentor, Brunswick, OH areas.

As a provider with Alteas Health, you will deliver cardiology-focused care within senior care facilities, managing a consistent patient volume while maintaining quality and efficiency.

This role is ideal for a provider who is comfortable working in a structured, high-productivity environment and takes pride in staying organized, decisive, and clinically sharp throughout the day.

What You’ll Do

• Evaluate and manage acute, chronic, and episodic conditions
• Develop and implement patient-centered care plans
• Order, perform, and interpret diagnostic testing, including EKGs
• Prescribe medications and recommend appropriate interventions
• Educate patients and facility staff on treatment plans and disease management
• Maintain accurate, timely, and compliant documentation
• Complete and oversee the patient consent process

Salary Range: $120,000 – $140,000

What We’re Looking For

Minimum Requirements:

• Active and unrestricted, or pending NP license in OH
• Reliable, insured transportation
• Minimum of 6 months of provider experience

Preferred Experience:

• 2+ years in a nursing home or post-acute setting
• Cardiology experience
• 2+ years of provider experience
