The CentraCare Senior Transitions team brings comprehensive, patient-centered care directly to individuals living in assisted living, memory care, long-term care, and transitional care settings — meeting patients where they are, in every sense of the phrase.
Summary
Our interdisciplinary team works closely with patients, families, and fellow healthcare providers to deliver care that is preventative, proactive, and tailored to each person's unique needs. Together, we manage chronic conditions, address social determinants of health, support care transitions to and from inpatient settings — all within an integrated, collaborative model.
As a provider on this team, you'll travel to assisted living, memory care, and long-term care communities across Central Minnesota, supported by a dedicated team of clinical operations and administrative colleagues working remotely alongside you. It's a role that offers autonomy, variety, and the deep satisfaction of building meaningful relationships with some of the patients who need it most.
