PACE Physician Assistant

Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living.
PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.
Centra’s PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our care model. This is a unique opportunity to be part of an innovative program where comprehensive care is delivered all under one roof.
If you value collaboration and are passionate about team-based care that prioritizes people over numbers, consider making PACE your home.

Why Farmville?
The Town of Farmville is a destination for tourism, recreation, retirement, and trade not only because of its attention to infrastructure but also because of its attention to its beauty. It is attractive to those who live, work or visit here. Its rich history and status as a center of trade and commerce are preserved and enhanced by its dedication to architectural integrity and improvement of the natural environment through a system of parks, green spaces, walking trails, and waterways. New growth and development of the Town incorporate the concepts of history, civic pride, and natural beauty. The Town supports a thriving downtown and five shopping centers.  Farmville is "open for business" and receptive to innovation, yet mindful of the responsibility to preserve its historic and natural resources for future generations.
