St. Peter’s Health Partners is seeking a full time Nurse Practitioner or Physician Assistant to join our Community Palliative Care Team, providing patient centered care in the home throughout the Capital Region. Our community based palliative care program focuses on improving quality of life for individuals living with serious illness by providing expert symptom management, compassionate support, and guidance through complex medical decisions.

Position Details:

  • Full time Community Palliative Care NP/PA role providing in home care to individuals with serious or chronic illness
  • Home based schedule with start/end from home, no weekends or holidays, and minimal on call
  • Conduct comprehensive assessments and develop individualized, goal aligned care plans
  • Manage pain and complex symptoms, including physical, emotional, and psychosocial needs
  • Provide patient education, treatment options, and symptom management
  • Coordinate care across settings and collaborate with primary care, specialists, home care, and hospice

Requirements:

Applicants should have the following:

  • Board Certified Physician Assistant or Nurse Practitioner (NCCPA, FNP, AGPCNP, or AGACNP)
  • New York State license and DEA certification
  • Experience in palliative care, hospice, or oncology in preferred

Recruitment Package:

St. Peter’s Health Partners offers a comprehensive salary and compensation package that includes:

  • Compensation range: $138,729–$160,926
  • The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, or other forms of cash compensation as applicable to the position
