What You’ll Do

• Conduct annual risk assessments and chronic condition reviews via telehealth and in-home visits
• Schedule 12-14 telehealth visits or 6-7 in-home visits daily, depending on the assignment mix
• Coordinate and escalate urgent findings to patients’ primary care providers
• Perform assessments related to primary care for both pediatric and adult patients
• Follow up on patient care plans and maintain detailed documentation
• Participate in ongoing education during downtime and attend scheduled orientation
• Support patients in the Milwaukee area, performing home health visits as needed

What You Need
Required:

• Active Oregon Medical License
• Medcard credentialing confirmed
• 1+ years of experience in family practice or related outpatient setting
• Comfort with telehealth and home visits in a diverse patient population

Preferred:

• Experience with pediatric and adult patient care
• Familiarity with risk assessments and chronic medical condition reviews
• Strong communication and organizational skills
