What You’ll Do
- Conduct annual risk assessments and chronic condition reviews via telehealth and in-home visits
- Schedule 12-14 telehealth visits or 6-7 in-home visits daily, depending on the assignment mix
- Coordinate and escalate urgent findings to patients’ primary care providers
- Perform assessments related to primary care for both pediatric and adult patients
- Follow up on patient care plans and maintain detailed documentation
- Participate in ongoing education during downtime and attend scheduled orientation
- Support patients in the Milwaukee area, performing home health visits as needed
What You Need
Required:
- Active Oregon Medical License
- Medcard credentialing confirmed
- 1+ years of experience in family practice or related outpatient setting
- Comfort with telehealth and home visits in a diverse patient population
Preferred:
- Experience with pediatric and adult patient care
- Familiarity with risk assessments and chronic medical condition reviews
- Strong communication and organizational skills
