Woodridge Rehabilitation & Nursing at Central Vermont Medical Center (CVMC) is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our interdisciplinary team in providing high-quality care to a complex adult and geriatric population.
In this role, you’ll care for both long-term residents and post-acute patients recently discharged from the hospital, offering a unique blend of continuity and acuity within a collaborative, team-based environment.
What You’ll Do
Provide comprehensive inpatient care in a skilled nursing and rehabilitation setting
Manage a panel of approximately 6–8 patients per day
Assess and manage acute changes, chronic conditions, and post-hospital transitions
Lead daily rounding, including review and triage of overnight events with nursing staff
Support safe and efficient discharges back to primary care providers
Collaborate closely with physicians, nurses, rehab therapists, and care managers
Schedule
0.6 FTE (30 hours/week)
Three 10-hour shifts per week (typically 7:00am – 6:00pm)
Rotating weekend phone call (7:00 AM – 7:00 PM, from home); paid call coverage

Qualifications
NP or PA with (or eligible for) Vermont licensure
BLS certification required
Preferred:
2+ years of experience in primary care, hospital medicine, urgent care, or post-acute care
Comfort managing medically complex adult or geriatric populations
Ability to work independently while collaborating closely with a multidisciplinary team
Given the complexity of the patient population, candidates with prior APP clinical experience are strongly preferred
Education
Nurse Practitioner (NP)
Graduate of a Nurse Practitioner program (Master’s or Doctoral degree) approved by the Vermont Board of Nursing or national accrediting agency
Maintains active licensure as a registered nurse (RN) and active, unrestricted licensure to practice as an Advanced Practice RN (APRN) by the Vermont Board of Nursing
RNFA certification strongly recommended
DEA registration (or eligibility)
Physician Assistant (PA)
Successful completion of a Physician Assistant program at an institution accredited by ARC?PA (Accreditation and Review Commission on Education for the PA)
Bachelor’s degree required; Master’s degree preferred
Current certification by the National Commission on Certification of Physician Assistants (NCCPA)
Active, unrestricted licensure to practice as a PA by the Vermont Board of Medical Practice
