Bridge Home Health & Hospice

Join a collaborative, mission-driven Palliative Care team dedicated to improving quality of life for patients with serious illness. As part of our established Bridge Hospice & Palliative Care program, you'll partner with experienced physicians, advanced practice providers, and an interdisciplinary team to deliver compassionate, patient-centered care in the inpatient setting. This is an opportunity to make a meaningful impact by supporting patients and families through complex medical decisions while working in a collaborative and supportive environment.

Position Highlights

  • Full-time inpatient position
  • 75% at Blanchard Valley Hospital (Findlay, OH) and 25% at Lima Memorial Hospital (Lima, OH)
  • Monday-Friday schedule (8:00 AM-4:30/5:00 PM)
  • Shared evening and weekend telephone call rotation
  • Collaborate with experienced Nurse Practitioners, Hospice Medical Director, and Palliative Care Medical Director
  • New graduate Nurse Practitioners with a passion for palliative care are encouraged to apply

Responsibilities

  • Perform inpatient palliative care consultations and follow-up visits
  • Manage symptoms for patients with serious, chronic, and life-limiting illnesses
  • Facilitate goals-of-care discussions and advance care planning with patients and families
  • Collaborate with hospitalists, specialists, nursing, case management, social work, and hospice teams
  • Assist with discharge planning, including transitions to home-based palliative care or hospice
  • Provide inpatient rounding for hospice General Inpatient (GIP) patients at Lima Memorial Hospital as needed
  • Participate in shared evening and weekend telephone call coverage
  • Document patient care using Cerner at Blanchard Valley Hospital, MEDITECH at Lima Memorial Hospital, and Homecare Homebase for hospice services

Qualifications

  • Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
  • New graduate Nurse Practitioners encouraged to apply
  • Strong communication, interpersonal, and organizational skills
  • Compassionate, patient-centered approach to care
  • Comfortable navigating complex conversations related to serious illness, prognosis, goals of care, and end-of-life planning
  • Ability to work independently while collaborating within an interdisciplinary team

Bridge Palliative Care

Bridge Palliative Care, a division of Blanchard Valley Medical Practices, provides comprehensive palliative care services for patients with serious illness. The team delivers inpatient palliative care at Blanchard Valley Hospital and partners with Lima Memorial Hospital to provide inpatient and outpatient services. Providers follow patients throughout their hospital journey, focusing on symptom management, care coordination, transitions of care, and ensuring treatment decisions align with each patient's goals and values.
