Location: New Haven, CT
Job Type: Full-Time (5 days/week) or Part-Time (Minimum 2 days/week)
Schedule: Monday – Friday, 8:30 AM – 5:00 PM (Flexible lunch options)
Our unique, patient-centric model blends scheduled appointments, same-day availability, and walk-ins primary care services to adult patients who prefer convenience. Our services include the acute visit, surgical clearances, weight management, physicals, preventative care and follow up care. We have been providing this care in West Haven for the past 14 years and our concept of care is growing.
We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team. In this role, you will manage a robust panel of adult patients, handling everything from preventative care to acute visits.
