Home-Based Nurse Practitioner - Gloucester County, NJ

• You are located in or near Gloucester County, New Jersey and able to travel locally for home visits.
• You have an active and unencumbered NJ Registered Nurse license.
• You are a Certified Advanced Practice Nurse Practitioner (APN).
• You have a master’s degree or post-master’s certificate in advanced clinical practice.
• You have a minimum of 1 year of NP experience in a clinical setting.
• You have active DEA and CDS registration.
• You have current Basic Life Support (BLS) certification.
• You have experience in home care or house call settings (preferred, but not required).

In this role, you will care for Medicare Advantage members with complex medical, functional, and social needs — many of whom are older, medically fragile, and at high risk for avoidable complications.

As a Field-Based Staff Clinician in the New Jersey Region, you will be accountable for the health outcomes of a defined panel of members. You will meet patients where they are, understand the realities of their home environment, identify risks early, and help coordinate the right care at the right time.
