Northeast Medical Group (NEMG), a member of Yale New Haven Health, is expanding Primary Care services throughout Connecticut and seeking experienced and new graduate Advanced Practice Registered Nurses (APRNs) and Physician Assistants (PAs) to join our growing team.

Position Responsibilities

As a Primary Care APP, you will:

• Provide comprehensive primary care services for patients across the lifespan
• Conduct patient assessments, develop treatment plans, and manage acute and chronic conditions
• Perform preventive health screenings and wellness visits
• Educate patients and families on disease prevention and management
• Collaborate with physicians and multidisciplinary care teams to coordinate care
• Utilize evidence-based practices to improve patient outcomes
• Document care accurately within the Epic electronic medical record system
• Participate in quality, safety, and population health initiatives

Qualifications

Required

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• National certification as an APRN or PA
• Connecticut APRN license or Connecticut PA license (or eligibility)
• DEA license or eligibility
• Excellent communication and interpersonal skills
• Commitment to patient-centered care and teamwork

Preferred

• Primary Care, Family Medicine, Internal Medicine, or Urgent Care experience
• Interest in preventive medicine and chronic disease management
• Experience working within a team-based care model
