*Responsibilities:*
* Conduct comprehensive assessments of patients' physical abilities and limitations.
* Develop and implement individualized therapy plans to address specific conditions and enhance physical function.
* Provide direct therapy services to clients in individual and group settings.
* Educate patients and their families on techniques to improve physical function and prevent injury.
* Monitor patients' progress and adjust therapy plans as needed to promote optimal outcomes.
* Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
* Document patient care accurately and maintain confidentiality of patient information.
* Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.
* Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
*Physical Therapist Qualifications:*
* Current Physical Therapist (PT) license in the state of CA.
* 1-2 years of clinical experience in physical therapy, preferably in Home Health or a similar setting.
* BLS/CPR certification required.
* Valid CA Driver's license.
Pay: $130,000.00 - $170,000.00 per year
