Key Responsibilities:
- Manage patients with advanced heart failure, including medication optimization (GDMT, diuretics)
- Order and interpret labs, echocardiograms, and cardiac imaging
- Educate patients and families on disease management and treatment options
- Support heart transplant evaluations and present cases in multidisciplinary meetings
- Collaborate with physicians, transplant coordinators, and care teams
Requirements:
- NP or PA, board certified
- Maryland license (or eligible), DEA as applicable
- ACLS/BLS required
- Cardiology, heart failure, or transplant experience preferred
- Strong clinical judgment and team-based care skills
Schedule:
- Outpatient-focused role
