Seeking Palliative Care NP or PA - Mercy Health Lima, OH

Mercy Health – St. Rita’s Medical Center is seeking a nurse practitioner or physician assistant for our growing Palliative Care Service Line. The provider will work with our palliative care physician and current APC on both the inpatient and outpatient setting.  This is a is rapidly expanding practice.

Education and Skills Required:

  • Certified by the NCCPA for Physician Assistant
  • Certified by AANP or like certification for Nurse Practitioners.
  • Current licensure with the state of Ohio as a PA-C or CNP.
  • Good verbal and written communication skills.

Position Highlights:

  • Ensure that all palliative care patients admitted in the acute care setting receive a navigation plan upon discharging back into the community.  
  • Build a patient visit census in the home based and extended care facility settings. 
  • May be consulted by home health nurses for patients who are admitted in the hospital who require a skill and are admitted to the Home Health Program. 
  • Assure timely and appropriate referral to hospice for patients in the PC Community based program are at the terminal stage of their disease. 
  • Educate the community ECF staff and leaders and assist with consults in the acute care setting.
  • Work toward the goal of having the capacity to see PC patients in any area of the community setting where Mercy Health is involved.
  • Increase the continuity of care with PC patients being discharged from the acute care setting with an individualized care plan.
  • APC to become an expert in the field of hospice and PC in order to increase community awareness of the services and benefit of this care.
  • Collaborate with the Hospice and PC physician and educate the medical professionals within Mercy Health to raise awareness of services offered.
