Nurse Practitioner or Physician Assistant – Home-Based Primary Care

We are seeking a Nurse Practitioner or Physician Assistant who resides in Ocean, Monmouth, or Mercer County, MJ, to join our Toms River, NJ medical group and help us continue our mission of transforming care for vulnerable populations.

Position Overview

As a HarmonyCares NP or PA, you will provide in-home primary care to a panel of chronically ill adult patients. This is an opportunity to practice medicine the way you envisioned—unhurried, relationship-based, and focused on whole-person care.

What you can expect:

• Manage a smaller patient panel to allow for meaningful, 30–60-minute visits

• Provide proactive, preventive care aimed at reducing ED visits and hospitalizations

• Develop care plans, coordinate with the interdisciplinary team, and build long-term patient relationships

• Enjoy strong clinical support from physicians, care managers, social workers, and in-home service teams

• Work independently with the backing of a national organization committed to quality and compassion

Qualifications

• Current NP or PA licensure in New Jersey

• Board certification

• DEA license or ability to obtain

• Valid driver’s license and reliable transportation

• Experience with geriatric, home-based, or complex-care populations preferred but not required
