HarmonyCares is seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care to patients in their homes throughout Passaic County, New Jersey, including the Paterson area.

What the role looks like

• Full-time, Monday–Friday schedule
• Care for an established panel of patients in and around Passaic County and the Paterson area
• Typically 5–8 patient visits per day
• Longer, more meaningful visits — often 30–60 minutes
• Smaller patient panels and manageable daily visit volume
• Provide comprehensive primary care, including assessment, diagnosis, treatment, medication management, and chronic disease management
• Collaborate with physicians, APPs, clinical support teams, and operations partners
• Practice within a value-based care model focused on quality and outcomes rather than visit volume
• Patient visits are geographically clustered to help minimize unnecessary travel
• Technology and clinical support designed to give providers more time to focus on patient care
• Limited after-hours telephonic call shared across the provider team

Qualifications

• Current New Jersey license as a Nurse Practitioner or Physician Assistant
• Appropriate national board certification
• DEA registration or ability to obtain
• Prior experience practicing as an NP or PA; this position is not intended for new graduates
• Primary care, internal medicine, family medicine, geriatrics, or similar experience preferred
• Comfortable traveling locally to patients' homes within the assigned Passaic County territory
• Spanish-speaking or bilingual English/Spanish strongly preferred
