Primary Care Nurse Practitioner
Location: Chesapeake, Virginia
Practice: Chesapeake Internists Limited
Position Overview:
Chesapeake Internists Limited, a well-established internal medicine practice in Chesapeake, Virginia, is seeking a dedicated Primary Care Nurse Practitioner (FNP or AGNP) to join our team and provide comprehensive primary care services to adult patients aged 18 and older.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for adult patients.
- Manage common acute and chronic conditions, including hypertension, diabetes, and respiratory illnesses.
- Perform routine physical examinations and preventive health screenings.
- Provide patient education on health maintenance, disease prevention, and lifestyle modifications.
- Conduct women's health screenings, including Pap smears and pelvic exams.
- Collaborate with physicians and other healthcare professionals to ensure coordinated care.
- Utilize electronic health records (EHR) for accurate and timely documentation.
Qualifications:
- Education: Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
- Experience: Experience preferred, but new graduates are welcome to apply.
- Licensure: Active Virginia license or eligibility to obtain.
- Skills: Strong clinical assessment, communication, and interpersonal skills.
