• Transition clinic helps Family Medicine focus on continuity and preventative care instead of just acute post discharge stabilization. 
• Highly competitive compensation model that includes incentives for production, panel size, quality/value-based goal achievement, and additional compensation for oversight of Advanced Practice Providers. Guaranteed minimum compensation for the first two years
