This is a growth-focused role with a strong opportunity to help expand services, shape workflows, and build the future of geriatric care across the region.

The position provides care across 7 assisted living facilities and a hospital-based SNF/rehab unit, supporting approximately 234 assisted living patients and ~12 SNF patients (average daily census). The physician will work within a collaborative, supportive team that includes an APP, CMA, administrative support, and strong partnership with local psychiatry services to support a complex behavioral health population.

Key responsibilities include:

  • Providing comprehensive primary care to geriatric patients in assisted living and SNF settings
  • Managing chronic disease, acute care needs, and preventive care
  • Conducting facility-based visits across 1–2 sites per day – less than 20 minutes commuting distance from main location.
  • Coordinating care across facilities, hospitals, and specialty providers
  • Building strong relationships with patients, families, and facility staff
  • Supporting and helping grow the program, including workflow development and quality initiatives

Practice highlights:

  • ~10 patients per day in a community-based setting
  • Shared home-based call across Transitional Care, Hospice, and Palliative teams
    • Weeknights: 6 PM – 6 AM (rotating)
    • Weekends: 1:9 rotation
  • Collaborative, team-oriented environment with strong system support
  • Interactions with residents given vibrant Internal Medicine Residency
  • Opportunity to precept and teach
  • Focus on continuity, compassion, and relationship-based care
  • Robust onboarding period with support from physicians and advanced practitioners

Ideal candidate:

  • Board-certified/board-eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Passion for geriatric and underserved populations
  • Strong communicator and team player
  • Interested in helping grow and develop a rapidly expanding service line
  • Experience in assisted living, SNF, or home-based care preferred

This is an excellent opportunity to join a mission-driven organization in the scenic Shenandoah Valley while playing a key role in building and expanding a growing geriatric care program within a strong integrated health system.
