Geriatric Physician – Outpatient Primary Care

Augusta Health is seeking a dedicated Geriatric or Internal Medicine Physician to join our Transitional Care team, providing high-quality, patient-centered care to residents in assisted living facilities. This full-time, outpatient position supports seniors in aging with dignity while remaining in their home environments.

Position Highlights:

• Outpatient-only care in local assisted living communities

• Average 5–10 patients per day

• Supported by experienced APPs, case managers, and administrative staff

• Shared call rotation with predictable scheduling
