Yale New Haven Health and Primary Care at Masonicare are seeking a Board Certified/Board Eligible Internal Medicine or Family Medicine Physician to join our growing primary care team in Wallingford, Connecticut.

Position Responsibilities

As a valued member of our primary care team, you will:

  • Provide comprehensive outpatient care to patients across the lifespan
  • Conduct detailed medical histories and physical examinations
  • Order, interpret, and evaluate diagnostic studies
  • Diagnose and manage acute and chronic medical conditions
  • Develop individualized treatment plans focused on preventive care and disease management
  • Prescribe medications and coordinate therapeutic interventions
  • Educate patients regarding health maintenance, wellness, and lifestyle modifications
  • Manage chronic diseases such as hypertension, diabetes, asthma, and hyperlipidemia
  • Evaluate and treat common acute illnesses and injuries
  • Perform minor office-based procedures within scope and training
  • Coordinate referrals and collaborate with specialists throughout Yale New Haven Health
  • Maintain accurate and timely documentation within the electronic medical record
  • Participate in quality improvement and population health initiatives
  • Support a culture of teamwork, excellence, and patient-centered care

Qualifications

Required Education and Training

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Completion of an ACGME-accredited Internal Medicine or Family Medicine residency program

Preferred

  • Board Certified or Board Eligible in Internal Medicine or Family Medicine
  • Strong commitment to team-based, patient-focused care
  • Excellent communication and relationship-building skills
  • Interest in preventive medicine and population health

Licensure Requirements

  • Board Certification or Board Eligibility in Internal Medicine or Family Medicine
  • Valid and unrestricted Connecticut medical license or eligibility to obtain one
  • Current DEA registration or eligibility to obtain
  • Connecticut Controlled Substance Registration or eligibility
  • Current BLS certification
