Hospital at Home Physician

Location: Winston-Salem, North Carolina

Department/ Section: Internal Medicine, Hospital Medicine – Hospital at Home Team

Position Summary

The Hospital at Home Physician at WFBH will be a part of the Section of Hospital Medicine at AH WFB. The Hospital at Home Physician will be responsible for patient care through the Hospital at Home program and other innovative telemedicine that comes through this program. The HaH program provides virtual acute care to patients and is supported by paramedics, Nurse navigators and other ancillary staff. The Hospital at Home physician will be expected to provide supervision to credentialed Advanced Practice Clinicians (APC) who are providing care to patients through the HaH program.

 

Responsibilities

  1. Provide medical care to patients.
  2. Deliver virtual medical care and develop patient care plans using sound clinical judgment.
  3. Diagnose and treat diseases and injuries of the human internal organs.
  4. Diagnose and treat internal medical conditions via telehealth platforms.
  5. Examine patients (limitation expected due to virtual nature of the service provided).
  6. Conduct/order diagnostic tests.
  7. Interpret test results and prescribe treatments, including medications and therapies.
  8. Analyze the results and prescribe indicated treatment therapies including medications and Physical Therapies.
  9. Consult with other physicians, when needed.
  10. Collaborate with physicians and interdisciplinary teams to ensure coordinated care.
  11. Assist in patient education and goal setting.
  12. Provide patient education and participate in goal setting.
  13. Perform procedures as per departmental guidelines.
  14. Uphold AHWFB’s mission, values, and cultural commitments.
  15. Maintain timely and accurate medical documentation.
  16. Attend regular team meetings and contribute to continuous program improvement.
  17. Treats all members of the patient care team with respect and dignity.
  18. Complies with all policies and protocols of HaH and AHWFB.
  19. Attends HaH business meetings regularly.
  20. Brings a problem-solving attitude and is ready for ongoing change of a program built for flexibility.
  21. Provides patient centered care with dignity and compassion.
  22. Maintains and ensures patient confidentiality at all times.
  23. Reports to work on time and as scheduled.
  24. Attends annual review and performs departmental in-services.
  25. Represents the organization in a positive and professional manner.
  26. Attends committee, CQI and management meetings, as appropriate.
  27. Resolves staff concerns at the departmental level, utilizing the grievance process as required.
  28. Ability to work independently.
  29. Communicate effectively both verbally and in writing.
  30. Completes medical records in a timely manner as per Organization policies.

 

 

Qualifications

  1. MD or equivalent degree and License to practice in NC.
  2. Should be able to get credentialed at one of the AHWFB hospitals for inpatient care.
  3. Should maintain IP Hospital Medicine practice.
  4. 2 years of experience as a hospitalist. Outpatient experience in addition to 2 years as a hospitalist is a plus.
  5. Must maintain a DEA license.
  6. Treats all members of the patient care team with respect and dignity.
  7. Complies with all policies and protocols of HaH and AHWFB.
  8. Attends HaH business meetings regularly.
