Internal Medicine or Family Practice Physician – Post-Acute Rehabilitation

Harmony Health seeking dedicated and compassionate Internal Medicine or Family Practice Physicians to join our growing team in York, PA, providing high-quality care to patients in a Post-Acute Rehabilitation setting.

In this role, you will manage the care of patients transitioning from acute hospital settings to skilled rehabilitation centers. You will work collaboratively with facility staff, advanced practitioners, and care coordinators to improve patient outcomes and reduce readmission rates.

Position Responsibilities:
• Provide comprehensive medical care to patients in skilled rehab facilities
• Conduct admissions, follow-ups, and discharge planning
• Collaborate with interdisciplinary teams including therapy, nursing, and case management
• Participate in care plan meetings and quality initiatives
• Ensure documentation and compliance with facility and regulatory standards

Position Requirements:
• MD or DO 
• Board Certification or Eligibility in Internal Medicine or Family Medicine
• Valid and unrestricted Pennsylvania medical license (or ability to obtain)
• Previous experience in post-acute, SNF, or rehab setting is a plus
• Excellent communication and clinical decision-making skills
