The Veterans Affairs Portland Healthcare System, Salem Healthcare Clinic in Salem, OR, is seeking full-time board certified/board eligible Family Practice or Internal Medicine Physicians to join our Primary Care Health Team. The primary care provider will function as a highly skilled, independent care provider in the management of patients, you’ll be part of an interdisciplinary care team with access to state-of-the-art medicine and some of the most advanced medical technologies available.
The Health Care Clinic operates under a PACT Model (Patient Aligned Care Team). Together with his or her team, comprised of a PCP, RN, LPN, and a Medical Support Assistant, the PCP cares for a panel of approximately 1200 patients. The PACT provides comprehensive care for our Veteran population, including the diagnoses, care, and treatment of both acute and chronic conditions. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. Position is Monday thru Friday, no weekends, and no holidays.
Although Salem is remote to the Portland VA, there exists multiple ties to the main facility. The Portland VA has a Community Based Out-Patient Clinic in Salem with a 26,000 square foot clinic, multiple primary care providers, and sub-specialty support. Veterans can receive consultations via electronic consults or Tele-health and may receive sub-specialty care through co-management with the private sector or via appointments at the Portland VA.
Of note, candidates must be board certified or eligible in Internal Medicine, Family Medicine or Geriatrics and have significant experience in the primary care of complex patients through interdisciplinary team management. Teaching experience is desirable.

As a teaching hospital, Portland VAMC provides a full range of services, with state-of-the-art technology as well as education and research.
Portland VAMC has an active affiliation with the Oregon Health & Sciences University. The Medical Center is connected both physically and functionally to the University. Over 1200 University residents, interns, and students are trained at the PVAMC each year. There are 172 base funded house officers in 33 different specialties.
