What You’ll Do
- Provide primary care to residents in skilled nursing / long-term care settings.
- Conduct assessments, diagnostics, and manage acute & chronic conditions.
- Develop care plans aligned with individual goals & coordinate with facility teams.
- Complete Medicare recertifications and required regulatory documentation.
- Mentor clinical staff; lead quality initiatives; implement best practices.
- Act as liaison between facility leadership and clinical teams for care coordination.
What You’ll Bring
- MD or DO (board certified or board eligible)
- Experience in post-acute care, long-term care, hospital medicine, or primary care
- Strong leadership, communication, and collaboration mindset
- Commitment to improving patient outcomes, preventing readmissions, and delivering compassionate care
- Eligible to be licensed in South Carolina
