Position Overview:

A 192-bed facility dedicated to providing compassionate and comprehensive care. Our services include long-term care, skilled nursing, short-term rehabilitation, respite care, hospice, and palliative care. We also provide specialty services such as stroke recovery, memory care, bariatric care, wound care, and VitalStim therapy. Qualifications:

  • Education: MD/DO
  • Licensure: Active NYS Medical License (good standing)
  • Certification: Board-certified in Internal Medicine, Family Medicine, or Geriatrics preferred.
  • Experience:
    • Demonstrated experience in long-term care, skilled nursing, or geriatrics.
    • Strong knowledge of CMS and NYSDOH regulations.
  • Soft Skills: 
    • Proven leadership and communication skills with the ability to work collaboratively with staff, providers, and administration.

Key Responsibilities

Resident Care & Oversight:

  • Provide guidance in the development, approval, and implementation of resident care policies.
  • Coordinate medical care and services, including participation in the Medical Advisory Committee (bi-monthly) and Executive QA Committee (quarterly).
  • Ensure attending physicians provide timely, appropriate care and follow required visit schedules.
  • Oversee infection control surveillance and review related reports.
  • Serve as physician of record for respite residents.
  • Provide second opinions or geriatric consultations as needed.

Provider & Staff Coordination:

  • Assist in credentialing and privilege reviews for physicians, dentists, and podiatrists.
  • Support communication and coordination among facility participating providers.
  • Lead meetings and provide education regarding facility policies, procedures, and regulatory updates (CMS/NYSDOH).
  • Provide physical examinations for new employees (included in monthly fee).

Quality Improvement & Compliance:

  • Monitor practitioner performance for compliance with standards of practice and regulations.
  • Participate in quality assessment and assurance programs, including reviewing incidents, pharmacist reports, and accidents.
  • Advise on the annual plan for clinical quality and patient safety.
  • Support staff and physician education to improve care and safety outcomes.

Leadership & Communication:

  • Report directly to the Nursing Home Administrator and/or DON/Designee.
  • Provide regular updates and collaborate closely with WCNH senior management.
  • Meet with administration at least every six months to maintain open communication and ensure program responsiveness.
  • Provide guidance and support to the Executive Director regarding clinical, operational, and strategic initiatives.
