Outpatient Primary Care Physician – CareMore Health

CareMore Health, part of Elevance Health, is seeking experienced Primary Care Physicians to join our outpatient team. We are reimagining healthcare delivery with a patient-centered, team-based approach that focuses on prevention, quality outcomes, and coordinated care.

Position Highlights

• Outpatient-only, full-time practice – no inpatient responsibilities

• Collaborative, team-based care model with nurse practitioners, case managers, behavioral health, and social services

• Administrative and clinical support so you can focus on patients, not paperwork

• Meaningful patient relationships with an emphasis on prevention and wellness

Qualifications

• MD/DO degree with Board Certification in Family Medicine or Internal Medicine

• Current or eligible medical license in the state of practice

• Minimum of 1–2 years of clinical experience in primary care required

• Commitment to patient-centered, value-based care
