Under the direction of the Chief Medical Officer, the Director of Health Information & Population Health (Medical Director) will lead efforts to leverage data and information systems to improve clinical outcomes and efficiency across Salud. This position oversees multiple Clinical Directors responsible for specific populations (Pediatrics, Women’s Health, Young Adult Health, etc.) and also provides direct patient care.

You’ll play a vital role in shaping Salud’s population health strategy — driving innovation, collaboration, and excellence in patient-centered care.

Key Responsibilities

Direct Patient Care

  • Provide comprehensive patient care including histories, exams, ordering/interpreting labs and imaging, prescribing, immunizations, and lifestyle guidance.
  • Perform procedures per documented training and privileges.
  • Collaborate with Salud’s services (OB, Pediatrics, Behavioral Health, Dental, etc.) for holistic care.
  • Refer patients appropriately and manage emergency situations.
  • Document all encounters promptly in the EHR.
  • Maintain licensure, CPR, and valid CA driver’s license.

Leadership

  • Support the CMO in leadership initiatives and monthly Board reports.
  • Lead and facilitate population health and PCMH meetings.
  • Partner with the Population Health Manager and collaborate with the COO and clinic managers to optimize patient flow.
  • Participate in quality, leadership, and management meetings.

Professional Conduct

  • Uphold Salud’s communication standards and model professionalism.
  • Communicate effectively with individuals from diverse backgrounds.
  • Promote collaboration across the healthcare team.
  • Recruit and onboard new providers.
  • Engage with professional societies and community partners to expand care reach.
  • Support quality improvement, policy development, and patient confidentiality compliance.

Management & Supervision

  • Supervise Clinical Directors for Pediatrics, Women’s Health, and Young Adult Health.
  • Collaborate with clinic managers and clinical staff for patient flow optimization.
  • May supervise students, residents, or coordinate placements.

Clinical Informatics & Population Health

  • Lead Salud’s population health initiatives and oversee data-driven decision-making.
  • Manage key information systems (EHR, practice management, electronic dental records).
  • Ensure data quality, validity, and compliance with reporting standards (UDS, HEDIS, Meaningful Use, etc.).
  • Oversee reporting for grants, incentives, and compliance programs.
  • Evaluate new technology to streamline operations and improve care.
  • Lead clinical quality improvement and chair QI committees.
  • Implement population management programs and improve care coordination.
  • Partner with the Executive Team on strategic initiatives.
  • Facilitate in-service training and technology integration for improved patient outcomes.

Requirements

  • Licensure: Valid CA Physician’s License, DEA License, and CA Driver’s License.
  • Certifications: Current CPR.
  • Education: Board Certified or Board Eligible in specialty preferred.
  • Preferred:
    • Master’s in Public Health, Clinical Informatics, or related field (or 2+ years of relevant experience).
    • 5+ years’ clinical experience, preferably within an FQHC.
    • Bilingual (Spanish/English).
    • Proficient in EHR systems and Microsoft Office Suite.
