Qualifications:

· Education: M.D., D.O.

· Board-certified or board-eligible in Family Medicine, Internal Medicine, or Pediatrics. 

· Completion of a Sports Medicine Fellowship, with a CAQ certification. 

· Strong clinical skills with a commitment to patient-centered care.

· Active, unrestricted FL license (or the ability to obtain FL license) is required
