Education, Training & Experience Required
- Board certified/board eligible Pain Medicine or Physical Medicine & Rehabilitation
- Fellowship trained preferred, but not required
- EMG training/experience preferred, but not required
- Licensed in the State of Wisconsin or willing to obtain license
