Some highlights of this opportunity include:

·       Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticut

·       Participate in the decision-making process in relation to admission to the hospice (homecare) or inpatient setting

·       Participating as a member of the Interdisciplinary Team in the development of individualized care plans

·       Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit

·       Consult with attending physicians regarding pain and symptom management

·       Coordinate efforts with the attending physician to provide continuity of care

·       Develop and monitor the medical component of the hospice program

·       Participate in the development, coordination and implementation of hospice services and policies

·       Participating in the development of staff and volunteer training

·       Conducts Face to Face visits for hospice admissions and recertification when appropriate as required by CMS. 

·       Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community

·       Perform related administrative functions as required and/or assigned

·       Participating on the Professional Advisory Committee

·       Establish health policies for employees when requested

·       Participate in the legislative process as it relates to hospice

·       Attend / participate in Hospice QAPI Team meetings and initiatives as requested
