• Description: Summary: General orthopedic surgeon needed for a 3-6 month assignment in Prescott Arizona to cover gaps of care prior to new providers onboarding. RATE CAPS FOR SUBMISSION: Malpractice – Must be included in rates. Orientation – 225 - half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” Hourly – 450 - “Per hour not OT” Holiday – 675 - Must be 1.5x the hourly rate. “Per holiday hour no OT” Nightly Call – 1550 - “Per night call to hold the pager to include 2.5 hours gratis” Holiday Nightly Call - 2325 - Per night call to hold the pager to include 2.5 hours gratis” On Call Daily – 3100 - “Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 4650 - “Per 24 hours to include 4 hours gratis Call Back – 450 – “Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Holiday Call Back – 675 – “Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Schedule/Availability Requirements: M-F amp Call Shifts Facility Location: 1001 Willow Creek Road Prescott Arizona 86301 Setting: Clinic/Surgery FTE: 1 Duties: Clinic Required Procedures: General EMR: Cerner Support Staff: MA/RN/Front Office Reason for Coverage: In between providers Deal breakers: N/A Travel Preferences: Preferred Lodging Air Travel: Credentialing Information: Timeframe: ASAP90 days possibly sooner.
