Malpractice
– Must be included in rates.
Orientation
– 200 - half the hourly/callback rate kicks in after 8 hours. “Per hour of
orientation or training beyond 8 hours”
Hourly – 400 - “Per hour not OT”
Holiday – 600 - Must be 1.5x the hourly rate. “Per holiday hour no OT”
On Call Daily – 3300 - “Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate - 4950 - “Per 24 hours to include 4 hours gratis
Call Back
– 400 – “Per hour of
patient contact hours after gratis hours are completed gratis prorate to MSA
terms.”
Holiday Call Back – 600 – “Per holiday hour of patient contact
hours after gratis hours are completed gratis prorate to MSA terms.”

Duties: Clinic After Hours Call Labor and Delivery Gynecologic surgery City Call CNM Back Up
Required Procedures: All standard gynecologic procedures in a clinic and hospital setting
EMR: EPIC
Support Staff: Front Desk and Clinical Support Staff including ultrasound on-site
Reason for Coverage: Physician Resignation
Deal breakers do not present this: if there is a history of concerns re: quality malpractice unable to clear no recent OB support
Travel Preferences:
Preferred Lodging: Preferred hotels Comfort Inn Holiday Inn
Air Travel: Local preferred but not required
Credentialing Information:
Timeframe: 60-90 days
