Position Title: Family Medicine Physician – Sonoma County Clinics (Full-Time)

Location: Santa Rosa, CA (Multiple Clinics in Sonoma County)

Position Overview:

We are seeking dedicated and experienced Family Medicine Physicians to join our network of clinics in Sonoma County. This full-time role offers an excellent opportunity to provide comprehensive primary care services across four distinct facilities, helping to meet the healthcare needs of diverse patient populations.

Key Responsibilities:

• Provide comprehensive primary care services including preventive, acute, and chronic care management for patients of all ages.
• Conduct thorough patient assessments, formulate diagnoses, and develop tailored treatment plans.
• Perform routine examinations and screenings, and interpret diagnostic tests to inform clinical decisions.
• Educate patients and families on health maintenance, disease prevention, and management strategies.
• Collaborate with multidisciplinary teams to ensure integrated and coordinated patient care.
• Maintain accurate and timely documentation of patient encounters in compliance with healthcare standards and regulations.
• Participate in quality improvement initiatives and professional development activities to enhance clinical practice.

Qualifications:

• Medical degree (MD or DO) from an accredited institution.
• Board Certification or Board Eligibility in Family Medicine.
• Valid and unrestricted California medical license.
• Proven experience in outpatient primary care.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a team-oriented environment.
• Commitment to providing patient-centered, culturally competent care.

Work Schedule:

• Full-time position, approximately 0.8 to 1.0 FTE, working across multiple clinics.
