• Description: Summary: Endovascular Neurointerventions Op Notes PostOp f/u MUST BE A NEUROSURGEON with a fellowship in ENDOVASCULAR! Rate CAPS for submission: Malpractice – Must be included in rates. Orientation – 275 - half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” On Call Daily – 5100 - “Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 7650 - “Per 24 hours to include 4 hours gratis Call Back – 550 – “Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Holiday Call Back – 825 – “Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Average worked Callback Hours: N/A Schedule/Availability Requirements:Ongoing coverage Facility Location: CHI Memorial - Chattanooga TNSetting: Acute Care/Inpatients/Interventional Neurology Lab/ORDuties: Procedure/Op Notes and Post NotesDays per month: TBDRequired Procedures: Endovascular Neuro Procedures Thrombectomies AneurysmEMR: EpicSupport Staff: Inpatient Hospital = APP 24/7 IN Lab = RN and RT both trained in Endovascular Neuro cases OR = normal OR contingent RN Surg Tech etc Dealbreaker: MUST be neurosurgeon with endovascular fellowship need someone to do surgeriesReason for Coverage: Comprehensive Intervention Coverage required 24/7 coverage for a Comprehensive Stroke Center Travel Preferences:Preferred Lodging: 1.5x GSA RateAir Travel: Local preferred but not required Credentialing Information:Timeframe: ASAPRequirements: Endovascular FellowshipCredentialing with other facilities?Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?
