Saratoga Hospital Geriatric Medicine is seeking a compassionate and dedicated Geriatrician to join our growing Geriatric medicine team within our IHI Age-Friendly Health System

This primarily outpatient position offers extended appointment times, fostering meaningful connections with patients and caregivers while delivering personalized care that aligns with individuals’ goals, values, and quality-of-life priorities. The primary clinical setting is an outpatient consultative geriatric assessment clinic and will play a key role in realizing the hospital’s Center for Successful Aging initiative. 

Key Responsibilities: 

  • Provide comprehensive outpatient geriatric assessments and consultative care for older adults with complex medical and social needs.
  • Collaborate closely with the hospital’s inpatient consultative geriatric service to ensure smooth transitions and continuity of care across settings. 
  • Work in a consultative role with community primary care providers, offering expertise in geriatric syndromes, polypharmacy management, functional assessment, dementia care, and advance care planning. 
  • Collaborate with other members of the Geriatric team including advanced practice providers, case management, social work and other interdisciplinary care teams to optimize patient outcomes through value-based, patient-centered team-based care.  
  • Opportunities for teaching family medicine residents and/or fellows exist based upon interest.

Qualifications: 

  • MD or DO with board certification/eligibility in Geriatric Medicine (Internal Medicine or Family Medicine trained). 
  • Eligible for licensure in New York State
  • Strong clinical skills in geriatric medicine, including diagnosis, management, and care planning for older adults. 
  • Commitment to value-based, patient-centered care models with a consultative focus. 
  • Excellent communication and interpersonal skills with patients, families, and healthcare professionals. 
  • Interest in innovation in Geriatric Medicine.
