Orientation - Orientation will consist of EMR training
Schedule - Mon- Friday 8-5
Setting - Outpatient
Rate Caps for Submission:
Malpractice – Must be included in rates. Orientation – half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” Hourly – 321-355 - “Per hour not OT” Duties - Consults manage inbox/results review for physicians who are not in the office. Patients/Day - 10-16
Support Staff - 1:1 ratio MA/MD
FTE: 1.0 open to part-time as well We would prefer to have one provider either doing part or full time.
Required Procedures: none
Thyroid biopsies - No
Patient Volume: 10-16 patients per day
Telehealth accepted in addition to onside coverage - Yes
Seeing new consults or follow ups only - New and Follow ups consults.
EMR: Cerner
Support Staff: Medical Assistants RN Front Desk Manager
Reason for Coverage: A physician left the practice in May and one other provider is going on leave.
Dealbreakers: Must have an Arizona License established.Credentialing Timeframe - 30 days

Credentialing with other facilities - No
Emergency credentialing - Yes we will request for an expedited credentialing if the physician is not credentialed with Dignity Health Medical Group. Travel Preferences see attachment for specific travel guidelines:
Preferred Lodging: Preferred hotels. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market. https://www.gsa.gov/travel/plan-book/per-diem-ratesAir Travel: Local preferred but not required. 750/round trip capRental Cars: 65/day all in cap including taxes and fees regardless of car class.Mileage: IRS Standard RatePlease detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Are procedures required? Thyroid biopsies? We are equipped with U/S guidance etc. to perform biopsies but it is not a requirement.
Will there be any call involved? Potentially yes I'm confirming with the onsite Manager after hours/weekend call rotation with 3 other providers for ambulatory clinic patients only.
