Parkview Physicians Group is currently seeking experienced Board Eligible / Board Certified Family Medicine or Internal Medicine physicians to join our Population Health team in Fort Wayne, Indiana.
Specifics of The Role
- Full time and part time opportunities available
- Hours are Monday-Friday from 8am – 5pm
- Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities
- Nurse practitioners cover call; physician would need to be available as a backup by phone
- Group consists of 1 physician & 5 advanced practice providers that are dedicated to providing quality continuation of care to patients that are discharged from the hospital
- Comprehensive care team and clinical support model comprised of medical assistants and registered nurses
