• 3-9 patients per day with extended 1:1 time
• Team based care with IDT meetings every morning at 8:30am
• Hospital and long-term care rehab rounding within 1 hour
• 1:4 call mostly telehealth, goes to experienced RN first
• Acute and episodic visits
