About Compass PLUS

Compass PLUS offers a distinctive model of primary care for patients with complex medical and social needs. Designed for physicians who thrive in high-need outpatient settings, the program combines relationship-based care, clinical flexibility, and a highly collaborative approach centered on each patient’s priorities.

Rather than relying solely on traditional office visits, the Compass PLUS team meets patients where they are—primarily in their homes and community settings. The physician works alongside a Nurse Practitioner, Nurse, Behavioral Health Clinician, and Community Health Worker to address medical, behavioral, and social barriers to care.

Most patients remain connected to their existing primary care physician, with Compass PLUS providing added support. For patients with the most intensive needs, the Compass PLUS physician becomes the primary provider, with the goal of stabilizing care and transitioning patients back to traditional primary care when appropriate.

About the Role

This is a unique opportunity to practice primary care with more time, flexibility, and support. You will manage a focused panel of approximately 100 medically and socially complex patients, serve as PCP for those fully transitioned into Compass PLUS, and consult with clinicians across the system on high-risk patient care. By practicing primarily in homes and community settings, you can identify barriers firsthand, build trusted relationships, and create care plans that reflect each patient’s circumstances.

Key Responsibilities

• Provide comprehensive, relationship-based primary care to a small panel of high-acuity patients
• Partner with a multidisciplinary team to deliver care in patients’ homes and community settings
• Develop individualized care plans that integrate medical, behavioral health, and social needs
• Consult with primary care physicians and care teams across the system on complex patient management
• Coordinate patient-centered transitions among inpatient, emergency, and outpatient settings
• Improve health outcomes and help reduce avoidable hospital and emergency department use

Practice Details

• Focused panel of approximately 100 patients, allowing time for intensive, personalized care
• Primarily home- and community-based care, with traditional office space available as needed
• Greater Boston–based practice
• Local travel required, with occasional regional travel approximately 1–2 times per month
• No evening or weekend call obligations
• Significant clinical autonomy within a supportive, team-based environment

Qualifications

• MD or DO with training in Internal Medicine or Family Medicine
• Board certified or board eligible
• Outpatient experience in high-need settings, such as community health centers or home-based care, preferred
• Strong interest in population health and health equity
• Demonstrated success working within collaborative care teams
• Comfort practicing independently across varied care environments
• Eligible for Massachusetts medical licensure and DEA registration
