Position Highlights:

  • Position: EHMG Board Certified or Board Eligible Concierge Physician, Internal Medicine/ Family Medicine fellowship trained

  • Location: 600 W. Lake Cook Road, Suite 160, Buffalo Grove, IL 60089

  • Full Time - direct medical services for 40 hours per week, 46 weeks per year for full-time status (pro-rated for part-time status).

  • Easy access for patients through our health system, integrated via Epic EMR

  • Salary range: $220,001-366,623. The pay rate offered is determined within the pay range and is dependent on a variety of factors, including but not limited to FTE, prior experience, certifications, degrees, subspecialties, projected volumes, and quality performance measures

 

Qualifications:

 

Minimum Education Level

  • Doctor of Medicine with a current license to practice medicine in the state of Illinois

Minimum Licensure Requirements

  • Doctor of Medicine with a current license to practice medicine in the state of Illinois

  • Active DEA and Illinois Controlled Substance License

Minimum Certification Requirements

  • Board certification/Board eligible in Internal Medicine or Family Medicine

  • Current Basic CPR certification through American Heart Association.

Minimum Work Experience

  •  Minimum of 2 years of experience in area of expertise in Internal Medicine or Family Medicine.
