The Home Base Primary Care (HBPC) Physician is responsible for providing comprehensive medical services for veterans in their homes. The physician will provide patient care to veterans in the areas of primary care with a home-based focus. This role involves conducting home visits, developing personalized care plans, managing chronic illnesses, and collaborating with an interdisciplinary team to enhance the health and well-being of patients. Responsibilities Include:
Veteran-Centered Care:
- Conduct home visits to provide primary care services to veterans.
- Assess, diagnose, and manage acute and chronic health conditions.
- Develop and implement personalized care plans based on patient needs.
- Monitor and address changes in patient conditions, providing timely interventions.
- Educate patients and caregivers on healthcare management and preventive practices.
- Ensure accurate and thorough documentation of patient encounters.
Interdisciplinary Collaboration:
- Work closely with an interdisciplinary team, including nurses, social workers, and therapists.
Quality Improvement:
- Participate in continuous quality improvement projects to enhance the delivery of care.
