About this Opportunity: 

• Outpatient only, no OB, inpatient services covered by well-established Hospital Medicine Group
• Join a team of five family medicine physicians, three advanced practice providers, along with RN case manager, diabetes educator, behavorial health support and pharmacy
• Shared call (2-3 weeks annually)
