Family Medicine - Geriatric Medicine Physician Position in Portland, OR

  • Schedule: M-F, 8am-5pm (considering 3-4 days/week options)
  • Duration: Ongoing, minimum 2 weeks/month
  • Shift: 8 hours daily
  • Hours per Week: Approximately 32-40

  • Licensing: Active medical license, DEA required
  • Experience: Geriatric care experience preferred, BC or BE board certified in Internal or Family Medicine

What You’ll Do:
  • Provide outpatient geriatric care to adult patients over age 55
  • Manage patient cases in a clinic setting with support from a multidisciplinary team
  • Conduct off-site visits to patients’ residences, assisted living, or SNFs
  • Collaborate with an 11-person interdisciplinary team within a PACE program
  • Document patient care activities primarily through EMR systems
  • Participate in scheduled shifts with guaranteed hours and flexibility options
  • Support patient care coordination and community visits

What You Need:
  • Required:
    • Current, active medical license
    • DEA registration
    • BLS certification
    • Geriatric care experience
    • Board certification or eligibility in Internal Medicine or Family Medicine
  • Preferred:
    • Experience working with senior populations
    • Ability to drive to off-site patient locations

About the Assignment:
This position involves outpatient primary care for seniors within a community-focused PACE program. The provider will work alongside an interdisciplinary team, managing mostly adult patients over age 55. The role includes both clinic-based and home visits, supporting comprehensive elderly care in Portland, Oregon.
