Pay & Benefits:
- Hourly Pay Rate: $1.00/hour

What You’ll Do:
- Provide outpatient geriatric evaluations and management of chronic and complex conditions
- Develop and implement preventive and primary care strategies for elderly patients
- Coordinate care plans involving multiple health services to improve patient outcomes
- Document patient visits accurately and efficiently using Epic EMR
- Offer education and counseling on health maintenance and disease prevention
- Manage post-acute and ongoing health issues within a clinic setting
- Collaborate with interdisciplinary team members to optimize patient care
What You Need:
Required:
- Board Certification in Internal Medicine or Family Medicine
- Minimum 1 year of outpatient clinical experience in geriatrics or primary care
- Valid Texas medical license or eligibility for licensure
Preferred:
- Experience managing chronic conditions common among geriatric patients
- Strong communication and interdisciplinary collaboration skills
