Join our Team at the Department of Veterans Affairs Community Based Outpatient Clinic in Menlo Park, CA as a Physician - Community Living Center (Full-Time)!
Major duties include but are not limited to:
Clinical:
The Staff Physician is primarily assigned attending physician responsibilities in the Community Living Center (CLC). The physician independently provides comprehensive medical care to Veterans residing in the Community Living Center and is responsible for performing admission history and physical examinations, comprehensive assessments, diagnosis and development of management plans, referrals as deemed necessary, inpatient and outpatient consultations, supervision of Geriatric fellows, residents, medical students, and other rotating medical trainees, evaluating referrals for inpatient admission or consultation, leading and participating in family conferences, and serving as an active member of the interdisciplinary treatment team. The physician is responsible for the ongoing management of acute and chronic medical conditions, medication management, coordination of specialty consultations, advance care planning and goals-of-care discussions, discharge planning, and coordination of care to ensure high-quality, patient-centered, evidence-based geriatric care. The incumbent also participates in and may direct activities pertaining to quality improvement, patient safety, strategic planning, performance improvement initiatives, and preparation for and participation in unannounced surveys and accreditation activities conducted by Ascellon Health, The Joint Commission, and other applicable regulatory or accrediting agencies. The physician participates in the physician on-call schedule on a rotating basis with other physicians to provide after-hours, weekend, and holiday medical coverage for the Community Living Center and performs other related clinical duties as assigned by the Medical Director or Chief of Geriatrics & Extended Care Service.

The Inpatient Staff CLC Physician may hold a faculty appointment at Stanford University, in the division of GIM. The incumbent will participate in the educational activities of rotating medical fellows in Geriatrics and Palliative Care. In addition, the provider is expected to participate in the educational activities of trainees from multiple disciplines.
Research:
Participates in clinical research, quality improvement, and evidence-based practice initiatives as available and consistent with service needs.
