Questcare/Matrix, a division of Envision Physician Services, is seeking a Geriatrician for our established and growing Geriatric Consult Service at Medical City Plano in Dallas, Texas, to make an impact in the care of frail older hospitalized patients, especially those with cognitive impairment.

Details of the Position:
Cognitive impairment (specifically delirium) among older hospitalized patients is associated with serious negative consequences (such as loss of function, hospital complications, use of physical restraints, overuse of sedating medications, longer lengths of stay and even higher mortality). The role of the geriatric consultant in this position is to help prevent and manage delirium and thus improve outcomes associated with delirium through systems based interventions (such as the Delirium Unit*) and by directly helping these patients as a geriatric consultant. This is a true geriatric consultant position. You will not be the primary attending for patients. You will not directly be responsible for non-geriatric syndrome related medical problems. Just as a cardiologist, infectious disease specialist or nephrologist is consulted for complex cardiac, infectious or renal problems, respectively, you will be consulted for complex geriatric syndromes, **most important of which is delirium. The majority of consults are requested by our busy hospitalists (who care for the majority of patients), but some consults are requested by our intensivists, trauma surgeons and orthopedic surgeons. Note, almost all patients under the care of these latter three specialties are eventually cared for by the hospitalists. *Delirium Units are small specialized units (usually 4-6 beds), that are restraint-free, with higher than usual nurse/nurse aid:patient ratios. These nurses are specially trained in how to prevent and manage delirium and behaviors associated with cognitive impairment, using non-pharmacological interventions, especially early and frequent mobilization. Delirium Units can reduce negative consequences of delirium listed above.

**In addition to cognitive impairment, other common geriatric syndromes for which you may be consulted include but are not limited to: ground level fall with a hip fracture (helping with expediting surgical intervention and preventing peri-operative delirium),
goals of care in advanced dementia (e.g. feeding tubes).

-RVU based
-Hospital inpatient consults only position (no primary attending responsibilities of inpatients)
-Flexible schedule, but in general, Monday through Friday Consult Service, No weekends
-No nights
-Team: 1 other Geriatrician, 1 Nurse Practioner

Physician Requirements:
-Board Certified Internal Medicine / Family Practice
-Board Certified / Board Eligible in Geriatric Medicine
-Leadership skills, desire to impact changes on an individual patient basis and on a large scale hospital-wide basis, and an understanding and hands-on approach to working with all disciplines in the hospital setting.
